About the Role

We are seeking a highly motivated and compassionate Social Services Professional to join our dynamic hospital social work team at Bridgend County Borough Council.

The successful candidate will have a crucial role in providing support and assistance to adults and their families following admission into hospital.

As part of our Early Intervention & Prevention Hub, the hospital social work team works collaboratively with health professionals to undertake assessments and facilitate smooth, safe, and timely transitions from hospital to home or alternative care settings.

The team plays a pivotal role in enhancing the overall wellbeing of individuals by promoting a person-centred, strengths-based approach.

Key Responsibilities
  • Undertake comprehensive assessments to determine individual needs in line with the principles of the Social Services and Wellbeing Act.
  • Develop and implement care plans in collaboration with multi-disciplinary teams.
  • Facilitate access to community resources.
  • Undertake capacity assessments and address best interests.
  • Advocate for individual rights and services, ensuring ethical and legal compliance.
  • Support carers.
  • Work as a multi-disciplinary team and participate in multi-disciplinary meetings.
Requirements

The ideal candidate will have strong communication skills, enthusiasm, proficiency in multi-disciplinary working, and the ability to assertively build and sustain relationships with key partners.

The ability to greet customers through the medium of Welsh is a requirement for this post.

Protecting children, young people, or adults at risk is a core responsibility of all council employees.

A criminal records check by the Disclosure & Barring Service (DBS) is a requirement for this post.

A valid driving licence is a requirement for this post.
