Key Responsibilities
  • Assess patients' needs and develop personalized support plans to ensure their safe discharge from hospital.
  • Provide emotional and practical support to patients, including assistance with daily tasks, medication management, and accessing community resources.
  • Collaborate with healthcare professionals to ensure seamless communication and coordination of patient care.
  • Drive to different locations, including hospital, doctor surgeries, pharmacies, and patients' homes, using British Red Cross vehicles.
  • Update systems to track patient support and ensure accurate record-keeping.
Requirements
  • A manual driving license.
  • A caring and empathetic nature, with excellent communication and interpersonal skills.
  • Basic IT knowledge, with the ability to learn British Red Cross systems.
  • An understanding of the services provided by the NHS and the ability to offer guidance and advice to patients.
