About the Role

We are seeking a highly skilled and experienced professional to join our team as a Children Services Practitioner. This role will involve supporting the System Capacity and Flow Manager in managing patient flow from acute providers to and across SCFT Intermediate Care Units.

Key Responsibilities
  1. Ensure all bed capacity across the Trust is utilised while ensuring patients experience safe, high-quality care throughout their patient journey.
  2. Work collaboratively with external stakeholders and partner organisations to address urgent care local pressure, system surges, and escalation, ensuring capacity is fully utilised and patient flow is maximised.
Person Specification

We are looking for a candidate with a strong background in operational management, partnership working, and multidisciplinary team collaboration. The ideal candidate will have experience of service developments, change programmes, and project management.

The successful candidate will possess excellent communication and interpersonal skills, with the ability to engage and work collaboratively with a range of stakeholders. They will also have a calm and rational approach to situations where conflict is likely.

Requirements
  • Current NMC Registration or other Health Care Professional Qualification
  • Degree or equivalent experience
  • Evidence of continuous professional development
  • Relevant postgraduate level qualification or equivalent experience
