Job Description

The Field Care Supervisor is a key role within our organization, responsible for supporting the delivery of exceptional care services in the community. As a Field Care Supervisor, you will be working closely with care coordinators to plan and implement effective care worker rotas, ensuring that our clients receive the best possible care and support.

Key Responsibilities

  • Conduct regular reviews of individual services, including risk assessments and needs assessments.
  • Participate in multi-disciplinary reviews of service users' needs.
  • Shadow new care workers to ensure their competence and provide on-site observation and evaluation of care worker practice.
  • Work with care coordinators to plan and implement effective care worker rotas.

Requirements

To be successful in this role, you will need experience in carrying out initial risk and needs assessments, working in partnership with service users, their family, carers, advocates, healthcare professionals, and others to ensure the seamless delivery of holistic packages of individual care support.
