About the Role

We are seeking a highly skilled and experienced Health Visitor to join our team at The Royal Wolverhampton NHS Trust. As a Health Visitor, you will play a vital role in delivering high-quality healthcare services to young children and families in our community.

Key Responsibilities
  • Manage a designated caseload, providing universal preventative healthcare services to young children and families.
  • Develop and implement evidence-based programs to improve health outcomes for children and families.
  • Work collaboratively with multidisciplinary teams to ensure seamless care delivery.
  • Identify and address health needs of families and communities, using appropriate methods and tools.
  • Ensure a culture of continuous quality improvement through audit, patient/family/stakeholder feedback, and reflection on practice.
  • Act as a visible role model, creating a supportive ethos to empower staff to contribute to the delivery of high-quality person-centered care.
  • Participate in the appraisal process, identifying personal development needs that are consistent with the team, service, and organization's objectives.
