Job Summary

We are seeking a highly skilled and experienced Forensic Community Psychiatric Nurse to join our dynamic team at Cwm Taf Morgannwg University Health Board. As a key member of our Criminal Justice and Forensic Team, you will play a vital role in supporting individuals with mental health conditions within the criminal justice field.

Main Responsibilities
  • Provide expert care coordination and case management for a defined caseload of clients with complex and challenging needs and risks.
  • Take a lead role in the care planning process, ensuring that Service and CTMUHB policies are adhered to.
  • Participate in the evaluation of care at review meetings, case conferences, and ward rounds, including travel to private care providers in Wales and England.
  • Provide specialist knowledge and advice to other agencies for public protection purposes.
  • Attend relevant specialist assessment training and formulate risk management plans to ensure public safety.
  • Undertake necessary training and professional development to maintain professional standards and meet NMC registration requirements.

Requirements
  • Degree in Mental Health Nursing or equivalent skills, knowledge, and experience.
  • Experience of working within a CMHT/ORT environment, with knowledge of Care Co-ordination and the All Wales Measure 2010.
  • Ability to work in a challenging and changeable environment, with good communication and listening skills.
