We are seeking a highly skilled and experienced Forensic Community Psychiatric Nurse to join our dynamic team at Cwm Taf Morgannwg University Health Board. As a key member of our Criminal Justice and Forensic Team, you will play a vital role in supporting individuals with mental health conditions within the criminal justice field.
Main Responsibilities- Provide expert care coordination and case management for a defined caseload of clients with complex and challenging needs and risks, adhering to the Mental Health (Wales) Measure 2010.
- Take a lead role in the care planning process, ensuring that Service and CTMUHB policies are upheld.
- Participate in the evaluation of care at review meetings, case conferences, and ward rounds, involving travel to private care providers in Wales and England.
- Offer specialist knowledge and advice to other agencies for public protection purposes.
- Contribute to risk management planning and attend relevant training to enhance skills and knowledge.
- Engage in mandatory training as required and maintain professional development to meet NMC registration standards.
