Position Overview

We are looking for a dedicated Community Hub Coordinator to enhance our teams and assist service users within the Community Health Services for Nursing and Therapy. This role requires effective communication skills and the ability to work autonomously across various community environments. A flexible mindset and teamwork capabilities are essential, along with the initiative to operate independently.

Operational Hours

Our service operates from 08:00 to 18:00, seven days a week. Therefore, candidates must be adaptable and available to cover all shifts, which can range from 7.5 to 9.5 hours per day. Weekend and bank holiday shifts are also part of the role, with additional pay for these hours.

Essential Skills

  • Meticulous attention to detail.
  • Ability to manage multiple tasks simultaneously.
  • Strong telephone etiquette and communication skills.

Main Responsibilities

The role encompasses a variety of duties, including but not limited to:

  • General administrative responsibilities.
  • Utilizing the electronic patient record system.
  • Completing electronic referrals and associated tasks.
  • Assisting with data collection for auditing purposes.
  • Data entry tasks.
  • Providing support to Community Nursing and Therapy staff, Team Leads, and clinicians.
  • Managing the ordering and tracking of equipment and supplies.

The ideal candidate will be a collaborative team member with outstanding organizational and communication abilities.
