We are seeking a highly skilled and experienced Lead Worker to join our team at Trident Housing Association Limited. As a Lead Worker, you will play a vital role in providing early and targeted support to vulnerable adults, working flexibly across the city to support young people in their own accommodation.
Key Responsibilities- Undertake effective needs and risk assessments with individuals referred to the service, in accordance with the service's eligibility criteria.
- Develop co-produced, outcome-focused support plans that reflect each individual's personal strengths, goals, and aspirations.
- Deliver a flexible service, increasing or reducing the frequency and intensity of support as required, and re-introduce support to dormant cases should any tenancy or support issues arise.
- Support individuals to sustain accommodation, improve health and well-being outcomes, develop skills required to live independently, including offering practical support, maximizing income, budgeting, and addressing debt.
- Act as a community navigator, linking individuals into local services and activities to establish networks of support and reduce social isolation.
- Excellent communication skills and a customer-focused approach.
- Proven ability to work effectively with a wide variety of stakeholders, both statutory and non-statutory.
- Experience of agile working across locations to support a caseload of individuals.
- A working knowledge of legislation in regard to delivering support to young people.
- Experience in facilitating individual involvement.
- IT literacy and competence with word, excel, support planning software, and database management and creation.
- Maintaining accurate records that capture identified outcomes and Key Performance Indicators.
- Ability to manage competing priorities and work to deadline.
- Understanding of the wider strategic and legislative framework and associated outcomes.
Trident Housing Association Limited has over 60 years' experience of delivering homes, services, and innovation for up to 8,000 people in over 3,600 homes in some of the most disadvantaged communities in the Midlands.
The Group employs more than 750 staff, works with over 150 volunteers, and provides training places and apprenticeships for many more.
The Group has an annual turnover of more than £37m and owns assets close to £190m.
The Group consists of Trident Housing, which provides landlord services for the Group in respect of housing management and maintenance, and Trident Reach, which is a registered care and support charity delivering person-centered care and support services.
These services include supporting people within registered care, domiciliary care, people with learning difficulties, those with mental health needs, young people at risk, older people, BME groups, people at risk of being homeless, those seeking employability, education, and training opportunities, and those fleeing domestic abuse.
