About the Role
We are seeking a passionate mental health registered mental nurse to join our Harrow Triage and Assessment Team. As a Community Mental Health Practitioner, you will be responsible for providing high-quality care to patients with mental health needs in their local community.
Key Responsibilities
- Conduct comprehensive mental health assessments of patients presenting with a wide range of clinical conditions.
- Develop, implement, and evaluate programmes of care without supervision, unless otherwise indicated.
- Support and apply a holistic, person-centred approach to assessing the health and social care needs of people experiencing severe and enduring mental illness.
- Work collaboratively with multidisciplinary teams to provide comprehensive care packages for patients.
Requirements
- Current Professional Qualification in mental health nursing and registration with the professional body.
- Experience of working in multidisciplinary mental health teams with clients with acute/severe and enduring mental health problems.
- Excellent verbal and written communication skills, including report writing and presentations.
