We are seeking a skilled Mental Health Practitioner to join our Community Mental Health Services team in Harrow. As a Community Mental Health Practitioner, you will play a vital role in providing high-quality care to individuals with mental health needs in their local community.

Key Responsibilities

  • Conduct comprehensive mental health assessments of patients presenting with a wide range of clinical conditions.
  • Develop, implement, and evaluate programmes of care without supervision, unless otherwise indicated.
  • Support and apply a holistic, person-centred approach to assessing the health and social care needs of individuals experiencing severe and enduring mental illness.
  • Have a flexible, multi-disciplinary approach and a solid understanding of risk assessment, care planning, and the recovery model.
