About the Role

We are seeking a passionate and experienced Mental Health Practitioner to join our team at Central and North West London NHS Foundation Trust. As a Community Mental Health Practitioner, you will play a vital role in providing high-quality care to individuals with mental health needs in their local community.

Key Responsibilities

  • Conduct comprehensive mental health assessments and develop personalized care plans for patients with a wide range of clinical conditions.
  • Work collaboratively with multidisciplinary teams to provide holistic, person-centered care that addresses the physical, emotional, and social needs of patients.
  • Develop and implement programs of care that promote recovery, resilience, and well-being.

Requirements

  • Current Professional Qualification in mental health nursing.
  • Registration with the professional body.
  • Post-registration training and CPD.
  • Experience of working in multidisciplinary mental health teams.
  • Excellent communication and interpersonal skills.
