Job Title: Adult Social Worker

We are seeking an experienced Social Worker to join our Hospital Discharge Team on a temporary basis for an initial period of 3 months. This role is critical in supporting the safe and timely facilitation of hospital discharges, especially as the Acute Trust faces increased pressures, particularly during the approaching winter months.

Key Responsibilities:
  • Facilitate safe and timely hospital discharges by working closely with the Hospital Discharge Team and other health and social care professionals.
  • Manage a complex caseload of adults, ensuring that assessments and care plans are completed in a timely manner to support discharge.
  • Conduct person-centred assessments to determine the needs of individuals and their families, ensuring that discharge plans are tailored to meet identified needs.
  • Collaborate with multidisciplinary teams, including nurses, doctors, therapists, and community partners, to develop discharge plans that support patients' safe transition from hospital to home or other appropriate settings.
  • Provide professional guidance and support to service users and their families during the discharge process, ensuring that they are fully informed and involved in decision-making.
  • Maintain accurate records and documentation using Eclipse (knowledge of Eclipse is desirable).
  • Adhere to relevant legislation, policies, and procedures to ensure compliance and best practice in all discharge planning activities.
  • Respond promptly and effectively to changing priorities and pressures within the team, demonstrating the ability to work under pressure.
  • Drive to various locations as required to support patients and facilitate discharges.
Essential Criteria:
  • Qualified Social Worker with current registration with Social Work England.
  • Proven experience working within an Integrated Hospital Discharge Team, with the ability to hit the ground running.
  • Ability to manage a complex caseload and make sound decisions quickly and effectively.
  • Excellent communication, assessment, and organisational skills, with a commitment to collaborative working.
  • Knowledge of relevant legislation, including the Care Act, Mental Capacity Act, and Safeguarding Adults procedures.
  • Proficiency in using IT systems, including knowledge of Eclipse (desirable).
  • Full UK driving license and access to a car for work purposes.
