We are seeking a skilled and experienced Adult Social Worker to join our Hospital Discharge Team on a temporary basis. This role is critical in supporting the safe and timely facilitation of hospital discharges, particularly during periods of high demand.
Key Responsibilities:- Collaborate with multidisciplinary teams to develop discharge plans that support patients' safe transition from hospital to home or other appropriate settings.
- Conduct person-centred assessments to determine the needs of individuals and their families, ensuring that discharge plans are tailored to meet identified needs.
- Manage a complex caseload of adults, ensuring that assessments and care plans are completed in a timely manner to support discharge.
- Provide professional guidance and support to service users and their families during the discharge process, ensuring that they are fully informed and involved in decision-making.
- Respond promptly and effectively to changing priorities and pressures within the team, demonstrating the ability to work under pressure.
- Qualified Social Worker with current registration with Social Work England.
- Proven experience working within an Integrated Hospital Discharge Team, with the ability to hit the ground running.
- Ability to manage a complex caseload and make sound decisions quickly and effectively.
- Excellent communication, assessment, and organisational skills, with a commitment to collaborative working.
- Knowledge of relevant legislation, including the Care Act, Mental Capacity Act, and Safeguarding Adults procedures.
