We are seeking a dedicated and proactive Pulmonary Rehabilitation Community Support Specialist to join our dynamic team at Oxleas NHS Foundation Trust. This role is pivotal in delivering exceptional care to patients with chronic respiratory conditions within the community.

Role Responsibilities

  • Assist in the management of a diverse caseload of patients, implementing and enhancing treatment plans under the guidance of qualified physiotherapists.
  • Conduct thorough assessments of patients, including information gathering through collaboration with clients, caregivers, and the multidisciplinary team (MDT).
  • Administer field walking tests and gather outcome data using standardized measures to evaluate patient progress.
  • Provide recommendations for walking aids and other clinical equipment as delegated, ensuring accurate documentation of prescriptions.
  • Evaluate patients' inhaler techniques and provide necessary corrections to optimize their usage.
  • Monitor patient conditions, documenting responses to treatments and any significant changes in health status.
  • Facilitate group exercise sessions or independently supervise routine classes, adjusting activities based on patients' health conditions.
  • Organize and prioritize delegated tasks, demonstrating flexibility to meet the evolving needs of the service.
  • Implement treatment plans for adults with chronic lung conditions, assessing progress and making necessary adjustments.
  • Work autonomously at times, reporting progress and concerns back to the supervising physiotherapist.
  • Deliver individualized or group treatments across various settings, including community centers and patients' homes.
  • Plan and execute pulmonary rehabilitation sessions, ensuring high-quality support without direct supervision.
  • Guide clients and caregivers on exercise regimens, providing clear instructions and support.
  • Utilize strong interpersonal skills to foster a supportive environment for clients and their families.

Qualifications and Experience

Essential

  • Experience in health or fitness environments.
  • Strong organizational capabilities.
  • Excellent written and verbal communication skills.

Desirable

  • Experience in delivering group exercise sessions.
  • Ability to work independently.
  • Valid driver's license.
