Job Title: Care Manager

We are seeking a highly skilled and experienced Care Manager to lead our team in delivering exceptional care services to individuals with complex needs.

Key Responsibilities:

  • Oversee a team of care professionals in delivering person-centered care to individuals requiring 1:1 or 1:2 support.
  • Ensure compliance with regulatory requirements, including the Health and Social Care Act 2008.
  • Manage service costs effectively and maintain high-quality care standards.
  • Train and supervise team members to ensure they are equipped to meet evolving client needs.
  • Participate in key meetings and contribute to continuous learning and development.

Requirements:

  • NVQ Level 4 in Health and Social Care Leadership or equivalent.
  • Experience managing supported living care services in a community setting, including a team of staff, for individuals with complex diagnoses.
  • Strong leadership skills, with the ability to foster positive relationships and guide teams in supported living care settings.
  • Excellent communication and IT skills.
  • Proven track record in managing complex care packages and supporting individuals with mental health conditions, personality disorders, autism, and learning disabilities.
