About the Role

We are seeking dedicated and compassionate Social Workers to join our Adult Social Care team. Our mission is to empower adults to live independently, and we need your expertise to make this happen.

You will play a vital role in conducting assessments, managing complex cases, and providing high standards of care within our dynamic and diverse environment. This opportunity will challenge you to expand your skills and knowledge in the field of social care.

About Our Teams

Our Locality teams are renowned for crafting outstanding care plans that prioritize the needs and voices of service users. We integrate input from various professionals, families/carers, and external agencies to ensure the best possible outcomes. We operate within four distinct Locality teams aligned with the Primary Care Network (PCN) footprint - Wallington, Carshalton, Sutton & Cheam. These services cater to older people, individuals with physical and sensory disabilities, and those with mental health-related needs.

About You

We are looking for individuals with outstanding communication skills and a compassionate manner who can effectively manage high levels of emotional distress. Experience in adult care is highly valued, as is a passion for delivering person-centered care.

You will join a welcoming, supportive, and energetic team dedicated to providing first-class supervision and ongoing professional development opportunities.

Essential Requirements:

  • Registration with Social Work England
  • A degree in Social Care or a related field
  • Experience working within a multidisciplinary team setting
  • Experience assessing and managing risks in supporting residents
  • Sound knowledge and ability to deliver social work support to service users and their families
  • Excellent communication skills, both written and verbal
