About the Role

We are seeking a skilled and dedicated Adult Social Worker to join our team on a permanent basis. As a key member of our social care team, you will play a vital role in supporting and empowering adults who require social care services.

Key Responsibilities
  • Conduct Comprehensive Assessments: Conduct thorough assessments of adult's needs to identify the most effective care and support plans.
  • Develop Care Plans: Collaborate with individuals, families, and other professionals to develop and implement care plans that promote well-being and independence.
  • Safeguarding: Identify and address safeguarding concerns, ensuring the safety and protection of vulnerable adults in accordance with relevant policies and legislation.
  • Collaboration and Communication: Work effectively with individuals, families, and other professionals to deliver high-quality, person-centred care and support.
Requirements
  • Relevant Social Work Qualification: Possess a relevant social work qualification and a strong understanding of adult social care principles and practices.
  • Experience in Adult Social Work: Have at least 2 years of experience in adult social work, including assessment and care planning.
