Senior Community Mental Health Practitioner Opportunity

We are seeking a highly skilled and experienced Senior Community Mental Health Practitioner to join our team at Tees Esk and Wear Valleys NHS Foundation Trust.

The successful candidate will work alongside a multidisciplinary team delivering care coordination and psychosocial interventions to individuals experiencing a first episode of psychosis.

Main Responsibilities
  • Deliver evidence-based therapies and interventions to individuals with mental health needs.
  • Work collaboratively with the multidisciplinary team to provide comprehensive care.
  • Develop and maintain effective relationships with service users, families, and carers.
  • Contribute to the development and implementation of care plans.
  • Participate in clinical supervision and peer review.
Requirements
  • Post-graduate qualification in a relevant area.
  • Significant experience working with individuals with mental health needs.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Knowledge of evidence-based practice and care programme approach.
