About the Role

We are seeking a highly skilled and experienced Senior Community Mental Health Practitioner to join our team at Tees Esk and Wear Valleys NHS Foundation Trust. As a key member of our community mental health team, you will play a vital role in providing assessment and treatment to older adults living with mental health needs.

This is an exciting opportunity for someone who wants to develop their leadership and clinical skills in a community setting. You will be responsible for your own caseload, providing assessments, care coordination, and delivering interventions in the community with people who have complex needs.

Key Responsibilities
  • Provide assessment and treatment to older adults living with mental health needs
  • Develop and implement care plans to meet the needs of service users
  • Work collaboratively with other healthcare professionals to provide comprehensive care
  • Provide clinical supervision and support to colleagues
  • Contribute to the development and implementation of team policies and procedures
Requirements
  • Current professional registration with an approved professional body
  • Evidence of Continuing Professional Development
  • Post-graduate qualification in a relevant area
  • Significant experience working with people with mental ill health in a community setting
  • Leadership or management experience
