Senior Community Mental Health Practitioner

We are seeking a highly skilled and experienced Senior Community Mental Health Practitioner to join our team at Tees Esk and Wear Valleys NHS Foundation Trust. The successful candidate will work alongside a multidisciplinary team to deliver care coordination and psychosocial interventions to individuals experiencing a first episode of psychosis.

The role involves working with a high degree of autonomy and flexibility, with access to individual and group clinical, caseload and management supervision and appraisal. The ideal candidate will have a strong background in mental health, with experience of working with individuals with complex needs.

Main Responsibilities
  • Deliver care coordination and psychosocial interventions to individuals experiencing a first episode of psychosis.
  • Work alongside a multidisciplinary team to provide comprehensive care and support.
  • Develop and maintain effective relationships with service users, carers and other stakeholders.
  • Contribute to the development and implementation of care plans and risk assessments.
  • Participate in clinical supervision and appraisal to ensure high-quality practice.
Requirements
  • Significant experience working with individuals with mental health needs.
  • Strong knowledge of evidence-based practice and care programme approach.
  • Ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills.
  • Post-graduate qualification in a relevant area.
