Adults Mental Health Senior Practitioner

We are seeking a highly skilled and experienced Senior Community Mental Health Practitioner to join our team at Tees Esk and Wear Valleys NHS Foundation Trust. The successful candidate will work alongside a multidisciplinary team to deliver care coordination and psychosocial interventions to individuals experiencing a first episode of psychosis.
The role involves working with a high degree of autonomy and flexibility, with access to individual and group clinical, caseload and management supervision and appraisal. The ideal candidate will have a strong background in mental health, with experience of working with individuals with complex needs.
Main Responsibilities- Deliver care coordination and psychosocial interventions to individuals experiencing a first episode of psychosis.
- Work alongside a multidisciplinary team to provide comprehensive care and support.
- Develop and maintain effective relationships with service users, carers and other stakeholders.
- Contribute to the development and implementation of care plans and risk assessments.
- Participate in clinical supervision and appraisal to ensure high-quality practice.
- Significant experience working with individuals with mental health needs.
- Strong knowledge of evidence-based practice and care programme approach.
- Ability to work autonomously and as part of a team.
- Excellent communication and interpersonal skills.
- Post-graduate qualification in a relevant area.
