About the Role

This is an exciting opportunity to join the Dorset Clinical Commissioning Group as an ICRT Team Lead Poole Intermediate Care Specialist. As a key member of our team, you will be responsible for leading and managing the ICRT team in Poole, ensuring the delivery of high-quality intermediate care services to patients.

Key Responsibilities

  • Lead and manage the ICRT team in Poole, ensuring the delivery of high-quality intermediate care services to patients.
  • Develop and implement effective care plans for patients, working closely with multidisciplinary teams to ensure seamless care transitions.
  • Provide clinical supervision and mentoring to junior staff, promoting professional development and growth.
  • Evaluate data and produce reports using Microsoft Excel and Word, ensuring accurate and timely information for stakeholders.
  • Experience of using electronic patient/service user record systems, ensuring efficient and effective care delivery.
  • Plan and organize ongoing activities, including managing own time and that of junior staff and learners, ensuring effective time management and prioritization.
  • Demonstrate skills in written and spoken English to a standard that enables the post holder to carry out the full range of duties and responsibilities of the role effectively.

Requirements

  • Registered practitioner to degree/diploma level.
  • Membership of the relevant Professional Body.
  • Evidence of recent professional development in an up-to-date portfolio.
  • Learning and Assessing in Practice Qualification or equivalent practice assessors training.
  • Supervised experience gained during clinical placements.
  • Recent previous experience within a comparable role.
  • Demonstrable knowledge of assessment and therapeutic interventions in the area of practice.
  • Experience of developing programs of care for an individual or groups of patients/clients and of providing advice.
  • Ability to prioritize and organize workload effectively.
  • Experience of demonstrating duties to junior staff.
  • Desirable criteria: experience of providing clinical supervision and mentoring to junior staff.
