Key Responsibilities:
- Assess and monitor patient conditions, identifying and addressing any changes or concerns
- Develop and implement individualized care plans, working closely with patients, families, and other healthcare professionals
- Provide education and support to patients and their families, promoting independence and self-management
- Collaborate with multidisciplinary teams to ensure comprehensive and coordinated care

Requirements:
- Current registration with the Nursing and Midwifery Council (NMC)
- Minimum of 6 months paid experience in a Learning Disabilities setting within the last 3 years
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
- Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities
- Ability to work flexibly, including evenings, weekends, and bank holidays
