About the Role
Caring Homes Group is seeking a highly skilled and compassionate Registered Nurse to join our team as a Nurse Care Coordinator. As a key member of our clinical team, you will be responsible for delivering high-quality care to our residents, while also providing leadership and guidance to our care staff.
Key Responsibilities
- Develop and implement individualized care plans that meet the unique needs of each resident
- Conduct regular assessments and evaluations to ensure the highest level of care is being provided
- Provide guidance and support to care staff to ensure they have the necessary skills and knowledge to deliver exceptional care
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
- Participate in quality improvement initiatives to drive excellence in care delivery
Requirements
- Valid NMC registration as a Registered Nurse (RGN, RMN, or RNLD)
- Strong clinical skills and knowledge of care standards and regulations
- Excellent communication and interpersonal skills
- Ability to work collaboratively as part of a team
- Strong problem-solving and critical thinking skills
