We are seeking a permanent Band 5 registered nurse to join our Community Nursing Team at Campbeltown Hospital. Located on the Kintyre Peninsula, our team provides comprehensive care and caseload management, adopting a clinically effective approach to all aspects of practice.
Our Community Nursing Team delivers care throughout the peninsula, utilizing a holistic approach to assess, identify, and prioritize health needs within the home environment and wider community setting. We work closely with the primary health care team and colleagues within the Health and Social Care Partnership to ensure a coordinated approach to prevention of admission and hospital discharge.
Skilled clinical interventions, health education, and promotion are provided to maintain optimum independent living in the community. Applicants must be NMC registered and possess clinical assessment skills. Experience in palliative care is advantageous for this role.
