Under the direct guidance of the population health practitioner, the successful candidate will operate within their competencies and will be responsible for executing clinical components of NHS Health Checks.
Main ResponsibilitiesKey duties include, but are not limited to:
- Performing NHS health checks, new patient assessments, and health checks for individuals over 40.
- Conducting baseline observations such as pulse oximetry and blood pressure, ensuring accurate documentation of findings.
- Measuring Body Mass Index (BMI).
- Promoting health and well-being awareness, providing guidance to patients.
- Encouraging patients to adopt health promotion strategies and self-care principles.
- Delivering opportunistic health promotion and lifestyle advice.
- Assisting the practitioner in managing chronic diseases within a community context.
- Recording and evaluating blood pressure and hypertension.
- Screening for and directing individuals towards dementia resources.
- Providing accurate and relevant information to patients and groups within the scope of practice.
- Addressing health disparities in the community by advocating for equitable healthcare access and public health resources.
- Supporting the planning and execution of health promotion campaigns and initiatives, including screenings and awareness events.

About the CandidateIf you are a qualified and experienced healthcare assistant seeking a rewarding challenge, we would be eager to hear from you. The ideal candidate will possess the following skills and qualifications:
Essential Qualifications- GCSE A-D or grade 3-9 in Maths and English or equivalent.
- Level 3 or equivalent Regulated Qualification Framework (RQF) or National Vocational Qualification (NVQ) level 3 in Health and Social Care.
- Care Certificate.
- Experience in a healthcare environment.
- Conducting new patient medical assessments including height, weight, blood pressure, pulse, and wellness advice.
- Ability to maintain accurate clinical records.
- Basic skills in observation, including blood pressure, pulse, SATs, and blood glucose monitoring.
- Knowledge of clinical waste and sharps policies and procedures.
- Excellent communication skills, both written and verbal.
- Strong IT proficiency.
- Familiarity with patient record management systems, specifically SystmOne.
- Ability to work collaboratively within a team and independently.
- Good interpersonal skills.
- Vaccination and Immunisation Foundation Course for adults and children.
- Phlebotomy certification.
- Ear Syringing certification.
Rayleigh and District PCN is a collaboration of six GP practices serving approximately 56,000 patients in the Rayleigh, Hullbridge, and Hockley areas.
