About This Role
We are seeking a dedicated and experienced Senior Social Worker to join our team, providing vital support to older adults within our nursing home services.
As a Senior Social Worker, you will play a crucial role in ensuring the well-being and independence of our residents. You will work closely with families, multidisciplinary teams, and external agencies to develop and implement personalized care plans that address complex social needs.
Key Responsibilities:- Conduct comprehensive assessments of older adults' needs, identifying areas requiring support and intervention.
- Develop and implement individualized care plans, ensuring they align with residents' goals and preferences.
- Advocate for residents' rights and well-being, liaising with families, healthcare professionals, and relevant authorities.
- Provide specialist advice and guidance to nursing home staff on social work matters, promoting best practices and a person-centered approach.
