About the Role

We are seeking a highly skilled and experienced clinician to join our team at Uppingham Surgery as an Enhanced Diabetes Specialist. This is an exciting opportunity to work in a supportive and dynamic environment, providing high-quality care to patients with diabetes.

Key Responsibilities
  • Provide expert care and management to patients with diabetes, including monitoring, prescribing, and educating patients on self-care.
  • Work closely with our practice nurse and clinical teams to develop and implement individualized treatment plans for patients with long-term conditions.
  • Conduct annual diabetic reviews, making adjustments to treatment plans as necessary.
  • Deliver patient care, referring patients to secondary or specialist care when required.
  • Maintain accurate and up-to-date clinical records using ARDENS and SystmOne templates.
Requirements
  • Diploma or equivalent in diabetes management with knowledge of Insulin and GLP-1.
  • Professional registration with NMC, GPHC, or equivalent.
  • Independent Prescriber.
