About the Role

As a Community Reablement Worker, you will empower young people with special educational needs and disabilities (SEND) to achieve their full potential. You'll guide them in developing essential life skills, fostering independence, and integrating into their communities.

Your responsibilities will include:

  • Collaborating with individuals to establish personalized goals and create action plans for success.
  • Connecting young people with valuable community resources such as employment opportunities, volunteer programs, recreational activities, and social groups.
  • Providing support and guidance to navigate challenges and overcome obstacles.
  • Working closely with families and other professionals involved in the individual's care to ensure a coordinated approach.
  • Identifying potential risks and implementing strategies to promote safety and well-being.

  • A genuine passion for supporting young people with SEND.
  • Experience working with individuals with diverse needs and backgrounds.
  • Strong communication, interpersonal, and problem-solving skills.
  • A commitment to collaboration and teamwork.
  • Relevant qualifications or certifications in social work, education, or a related field.
