About the Role

We are seeking a highly skilled and experienced Community Dietitian to join our dynamic team at Mid Cheshire Hospitals NHS Foundation Trust. As a Community Dietitian with expertise in Home Enteral Feeding, you will play a vital role in delivering specialist dietetic services to adults requiring enteral feeding, prescribing support, or therapeutic dietary intervention across South Cheshire and Vale Royal.

Key Responsibilities
  • Deliver specialist dietetic services to adults requiring enteral feeding, prescribing support, or therapeutic dietary intervention.
  • Provide regular home visits, virtual clinics, and community dietetic clinics for patients requiring enteral feeding.
  • Formulate, deliver, and monitor nutritional care plans for patients, including complex clinical conditions and diet therapy.
  • Contribute to the development of clinical guidelines, resources, and education packages related to enteral tube feeding and Oral Nutritional Supplements (ONS).
  • Work closely with the multidisciplinary team, including community nurses, specialist nurses, and medicine management team, to provide comprehensive care to patients.
  • Develop and maintain strong communication and interpersonal skills to work effectively with patients, families, and healthcare professionals.
Requirements
  • HCPC registration as a Dietitian.
  • Competencies in enteral nutrition.
  • Experience in community dietetics, including enteral feeding.
  • Strong clinical skills in management of enteral feeding tubes.
  • Ability to work independently and as part of a team.
