Key Responsibilities

  • Collaborate closely with the Uro/Oncology Clinical Nurse Specialist team to assist patients in navigating the initial phases of a suspected cancer diagnosis through to treatment and recovery.
  • Provide support and information in both outpatient and inpatient environments.
  • Deliver comprehensive care and promote health for patients.
  • Conduct thorough needs assessments and coordinate with the multidisciplinary team.
  • The primary role of the Support Worker is to guide individuals through the intricate health and social care systems during and after cancer treatment.
  • Facilitate care coordination by serving as a single point of access, ensuring rapid re-entry into the system for those with urgent or specialized needs, and managing care, education, and support for individuals with non-complex care requirements.

Essential Qualifications

  • Demonstrated experience in a healthcare assistant or administrative role involving direct patient care within a healthcare setting at AFC band 3 or equivalent.
  • Experience in public interaction, both in-person and via telephone.
  • Proficiency in data management.
  • Good command of written and spoken English.
  • Experience in educating or coaching patients.
  • Strong organizational skills and IT proficiency.
  • Ability to take initiative and communicate effectively with individuals at all levels.
  • Excellent observational and practical problem-solving skills.
  • Capacity to work collaboratively within a team and motivate oneself and others.
  • Flexible approach to work responsibilities.
  • Empathy and understanding of the challenges faced by individuals affected by cancer.
  • Ability to prioritize tasks and sensitively inquire about information needs.
  • Capability to gather information from diverse sources and formats.
