About the Role

We are seeking a highly skilled and motivated Physiotherapist to join our Integrated Community Team in Gloucestershire Health and Care NHS Foundation Trust. As a Physiotherapist in this role, you will work closely with multidisciplinary teams to provide high-quality care to adults and older people in their own homes and within bed-based Reablement units.

Key Responsibilities
  • Conduct comprehensive assessments and develop personalized care plans to support patients in maintaining their health and wellbeing.
  • Work collaboratively with nursing, therapy, and Reablement teams to provide holistic care and promote hospital admission avoidance.
  • Use clinical assessment and reasoning tools to inform practice and ensure high-quality patient care.
  • Manage a caseload autonomously and make decisions about patient care in consultation with the team.
Requirements
  • HCPC registration as a Physiotherapist.
  • Relevant clinical experience in treating patients within a community MDT.
  • Evidence of autonomous working and managing a caseload.
  • Evidence of clinical assessment and reasoning tools in practice.
