Position Overview

An exceptional opportunity has emerged for dedicated and proactive professionals to become part of our Reactive Emergency Assessment Community Team (REACT) as a Lead Community Triage Nurse.

The REACT team is a collaborative, multi-disciplinary community service within the East Suffolk and North Essex NHS Foundation Trust, encompassing Urgent Community Response services, Outpatient Parenteral Antibiotics Therapy (OPAT), Out of Hours (OOHs) services, and Virtual Ward pathways for patients. This position involves working within the crisis team to assess and prioritize referrals for our Urgent Community Response Team, which has established innovative pathways with emergency services and community health initiatives.

Key Responsibilities

As a Lead Community Triage Nurse, your responsibilities will include:

  • Serving as the primary contact for new referrals to the REACT service, offering a telephone triage service to assess needs and prioritize interventions.
  • Being the initial point of contact for all REACT referrals, which may come from a variety of sources including healthcare professionals and self-referrals.
  • Conducting thorough telephone triage assessments utilizing exceptional listening skills, critical thinking, and clinical judgment.
  • Implementing a holistic approach to effectively allocate or direct referrals based on available information and service capacity.
  • Continuously monitoring service capacity, identifying discharges, and addressing any delays.
  • Actively participating as an innovative member of the multi-disciplinary team.
  • Prioritizing incoming referrals on an individual basis and coordinating with team clinicians as necessary.
  • Promoting and upholding high standards of care within the clinical governance framework.
  • Ensuring effective communication to provide the best possible experience and outcomes for patients in crisis, including conducting comprehensive handovers and presenting cases within the multidisciplinary team.
