Advanced Pharmacist Practitioner: Acute Care Specialist

We are seeking an experienced Clinical Pharmacist to join our team in the Emergency Department at Worcestershire Acute Hospitals NHS Trust. As a Clinical Pharmacist, you will play a key role in providing high-quality patient care and contributing to the development of our pharmacy services.
Key Responsibilities- Assess patients and identify medicine-related causes for attendance and admission
- Develop treatment plans to optimize care and improve patient outcomes
- Work independently or as part of a multidisciplinary team to deliver advanced clinical pharmacy services
- Provide specialist clinical pharmacy and medicines optimization services to patients
- Contribute to the development and implementation of quality improvement initiatives
- Professional knowledge acquired through a vocational degree in pharmacy
- Currently registered as a pharmacist in the UK and a member of the General Pharmaceutical Council
- Clinical specialist knowledge acquired through postgraduate level training or relevant experience
- Accredited independent prescriber
