Advanced Pharmacist Practitioner: Acute Care Specialist

We are seeking an experienced Clinical Pharmacist to join our team in the Acute Medical Directorate and Emergency Department at Worcestershire Acute Hospitals NHS Trust. As a Clinical Pharmacist, you will play a key role in providing high-quality patient care and contributing to the development of our pharmacy services.
Main Responsibilities- Assess patients and identify medicine-related causes for attendance and admission
- Develop treatment plans to optimize care and improve patient outcomes
- Work independently or as part of a multidisciplinary team to deliver advanced clinical pharmacy services
- Provide specialist clinical pharmacy and medicines optimisation services to patients
- Use your knowledge of medicines to advise consultants, medical, nursing, and pharmacy staff on pharmaceutical aspects of patient care

Person SpecificationWe are looking for a Clinical Pharmacist with the following qualifications and experience:
- Professional knowledge acquired through a vocational degree in pharmacy
- Currently registered as a pharmacist in the UK: Member of the General Pharmaceutical Council
- Clinical specialist knowledge acquired through postgraduate level training
- Accredited independent prescriber
We also welcome applications from candidates who have experience in delivering specialist clinical services to the relevant specialty or directorate, and who have a strong track record of influencing and persuading senior clinicians in relation to optimal medicine treatment for patients.
