About the Role

NHS Professionals Limited is seeking a skilled Community Psychiatric Nurse to join our team on a flexible shift basis. As a Community Psychiatric Nurse, you will play a vital role in delivering mental health support and interventions directly within the community.

Key Responsibilities

  • Provide robust assessments of patients who have been referred due to presenting with mental health difficulties.
  • Identify appropriate care pathways and ensure the least restrictive option to meet the needs of the patient and ensure their wellbeing.
  • Signpost to appropriate care providers, including primary and tertiary care.
  • Raise concerns to senior staff for support and management.
  • Complete documentation and planning care for patients.
  • Presentation cases to Multi-Disciplinary Team for discussion around care pathway.
  • Liaison with colleagues nursing, occupational therapy, medics, psychology and admin.
