About the Role
SW Healthcare is seeking a dedicated Care Coordinator to play a crucial role in coordinating care for residents of care homes. As a Care Coordinator, you will work closely with GP practices to provide seamless, patient-centered care.
Main Responsibilities
- Identify patients needing care packages and coordinate their support
- Develop and maintain personalized care plans in collaboration with patients, carers, and healthcare professionals
- Assist patients in managing appointments and accessing necessary support services
- Provide information and guidance to help patients make informed decisions about their care
- Coordinate care across various health and social care services
- Maintain accurate records and ensure timely updates to care plans
