Position Overview

The role involves contributing to the delivery of a high-quality Community Mental Health Service tailored for individuals experiencing early-onset or less common forms of dementia. This includes monitoring the mental health and cognitive well-being of service users, ensuring adherence to treatment plans, and facilitating engagement in daily living activities as part of a collaborative multi-disciplinary team.

Participation in team meetings and group supervision, along with individual supervision, is essential. It is crucial to report any observed changes in the service user's physical, cognitive, or mental health to the team manager while promoting a positive living experience for those with dementia.

Key Responsibilities

  • Primarily engage with clients in their residences, adhering to established safety protocols.
  • Collaborate with qualified personnel to assess client needs and develop suitable treatment strategies, which may involve direct observation and autonomous decision-making in challenging situations.
  • Assist in the practical aspects of care, showcasing appropriate person-centered care techniques to caregivers.
  • Contribute to the assessment and refinement of care plans through collaborative discussions and participation in team meetings.
  • Safely accompany clients to appointments and other necessary locations as outlined in their treatment plans.

Job Duties

  • Operate within the defined scope of the role, seeking guidance when necessary and reporting any issues to qualified staff members.
  • Comply with organizational policies, procedures, and guidelines.
  • Attend mandatory training sessions to ensure compliance and skill enhancement.
  • Engage in regular supervision sessions and annual performance evaluations.
  • Accompany clients as deemed appropriate by senior staff members.
  • Work closely with qualified staff to identify client needs and develop effective treatment plans, including periods of direct observation and assessment of factors contributing to challenging behaviors.
  • Model effective interventions to alleviate distress for clients and their caregivers.
  • Participate in the practical aspects of care, demonstrating person-centered approaches to caregivers.
  • Contribute to the evaluation of care plans through collaborative discussions and attendance at team meetings.
  • Safely escort clients to appointments and other locations as per their treatment plans.
  • Gather and compile information regarding clients' past experiences to assist caregivers in understanding the pre-morbid personality of the client, which can also serve as a diversion technique in challenging situations.

Essential Qualifications

  • Demonstrated understanding of dementia and person-centered care principles.
  • GCSE in Maths and English or equivalent qualifications.
  • Previous experience working with individuals with dementia in a mental health or care setting.
  • Proven ability to establish positive therapeutic relationships with service users and their caregivers.
  • Access to a vehicle or alternative means of mobility.
  • NVQ Level 3 or equivalent experience in a relevant field.

Desirable Qualifications

  • Experience in a community care environment.
  • Training in dementia care or NVQ Level 2 or 3 in Health & Social Care.
  • Familiarity with the Mental Capacity Act.
