Job Summary

We are seeking a highly skilled and experienced Clinical Pharmacist to join our team at Penryn Surgery. As a key member of our medicines management team, you will play a vital role in ensuring the safe and effective use of medicines within our practice.

Main Duties
  • Develop and manage a medicines management plan and deliver patient services as determined by practice policy and local and national guidance.
  • Develop and manage the medicines management team, including delivery of training, to maximise cost-effective prescribing and improve the quality of patient care.
  • Be the first point of contact for patients regarding use of their medicines and respond to queries from patients and healthcare professionals in a timely and professional manner.
  • Work with the wider team to develop and update relevant policies.

Responsibilities
  • Provide clinical supervision and professional leadership to the medicines management staff.
  • Be responsible for training medicines management staff to perform clinical audits around key areas of responsibility.
  • Provide medication review services to patients via clinics in the practice, domiciliary visits, and in residential and nursing homes, and deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care.
  • Produce patient information leaflets and posters and run medicine awareness projects throughout the year.
  • Assist Partners with the appropriate monitoring and management of their prescribing budgets.
  • Prepare evidence-based resources and information to support the medicine management team and all other relevant health professionals in the implementation of rational cost-effective prescribing.
  • Help plan, develop, and support the introduction of new working processes within the practice to optimise the quality of prescribing.
  • Implement and manage the repeat dispensing system for the practice, including developing, implementing paper prescribing systems within the practice using SystmOne, and reauthorising repeat prescriptions as a non-medical independent prescriber.
  • Manage repeat prescribing reauthorisation procedures by reviewing requests for repeat prescriptions and medicines reaching their review date.
  • Review discharge summaries received into the practice, including reconciling medicines following hospital discharge and working with patients and community pharmacists to ensure patients receive the medications they need following discharge and records updated accordingly on clinical systems.
  • Manage medicines queries from patients and healthcare professionals.
  • Implement drug alerts and withdrawals, for example, MHRA alerts.
  • Undertake Medicines Use Reviews to support medication review processes, including the identification and management of problematic polypharmacy.
Responsibility for Administration
  • Provide regular support and feedback to practice on prescribing action plans.
  • Produce quarterly reports on practice progress to the lead GP for medicines management.
  • Update and maintain accurate patient medication records on the practice's clinical computer systems, including advice given and action taken.
  • Advise the primary health care team on the safe and secure handling of controlled drugs and other medicines, ensuring compliance with medicines legislation.
Continuing Professional Development
  • Take responsibility for own development, learning, and performance, including participating in clinical supervision and acting as a positive role model.
  • Complete compulsory and mandatory training.
  • Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments.
  • Support and participate in shared learning across the practice and wider organisation.

  • Alert other team members to issues of quality and risk.
  • Assess own performance and take accountability for own actions, either directly or under supervision.
  • Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the team's performance.
