Role Overview:
As a Lead Practitioner for Healthy Living Services within the Healthy Living Service Team, you will play a pivotal role in driving positive change. Your commitment to collaboration will help shape a brighter future for families in our community.
Service Description:
The Healthy Living Service collaborates with children, young people, and their families facing challenges related to maintaining a healthy weight and lifestyle. We identify barriers that may hinder their progress and provide tailored interventions, including one-on-one support in various settings such as homes, community centers, and schools. Our team works closely with the Voluntary, Community, Faith, and Social Enterprise sectors to enhance our service delivery.
Key Responsibilities:
- Support Healthy Living Practitioners in planning and implementing evidence-based services for children and families.
- Ensure the effective delivery of Healthy Living Services to all identified families.
- Engage directly with children, young people, and families to promote sustainable improvements in their health and wellbeing.
- Act as the Lead Professional for designated families, addressing complex issues in collaboration with Social Workers and Health professionals.
- Maintain accurate and current records using electronic systems, adhering to local policies and guidelines.
- Provide information and guidance to families regarding relevant universal and specialized services, empowering them to take charge of their health.
Qualifications and Skills:
- A relevant graduate-level qualification or equivalent experience.
- Demonstrated knowledge and ability to support families on Healthy Living topics.
- Experience working directly with children and families in professional or voluntary roles.
- Understanding of the social and emotional factors influencing a child's development.
- Proficient IT skills to navigate various software applications.
