About the Role

We are seeking a highly skilled and compassionate Physiotherapist to join our Home Care team at MaineHealth Accountable Care Organization. As a Home Care Physical Therapist, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes.

Key Responsibilities
  • Assess patients' physical needs and develop personalized treatment plans to improve their mobility and function.
  • Provide evidence-based physical therapy interventions to promote optimal health outcomes.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient progress and outcomes to inform treatment decisions and quality improvement initiatives.
Requirements
  • Graduation from an accredited physical therapy program.
  • State of Maine licensure as a Physical Therapist.
  • Current, valid driver's license to operate a motor vehicle, with vehicle available for work covered by liability insurance as specified by agency policy.
  • Current certification in Basic Life Support for Healthcare providers.
  • Minimum one year of full-time equivalent experience in a supervised setting.
