Tameside Council is seeking a dedicated and compassionate Social Services Professional to join our Integrated Urgent Care Team. As a key member of our team, you will play a vital role in supporting adults to live well at home.
About the Role
This is an exciting opportunity to work in a fast-paced and rewarding environment, where you will have the chance to develop as a practitioner and access career progression opportunities. As a Social Services Professional, you will be responsible for managing a caseload, conducting Care Act Assessments, creating Support Plans, and signposting to internal and external services to enhance wellbeing and independence.
Key Responsibilities
- Manage a caseload of adult clients, providing support and guidance to enable them to live well at home.
- Conduct Care Act Assessments to identify the needs of adult clients and develop Support Plans to meet those needs.
- Work collaboratively with other disciplines and local health and social care services to provide a coordinated response to individuals and families.
- Signpost clients to internal and external services to enhance wellbeing and independence.
Requirements
To be successful in this role, you will need to have excellent communication skills, the ability to build effective professional relationships, and a passion for supporting adults to live well at home. Experience of working with adults and previous employment in a similar position would be desirable. It is essential that you hold a current valid driving licence and have access to a vehicle for work use.
