About the Role

We are seeking a skilled and compassionate Casual Support Worker to join our team in a Regulated Service. As an Assessment Support Worker, you will play a vital role in providing practical and emotional assistance to individuals in their own homes, assessing their needs and creating personalized care plans.

Key Responsibilities
  • Conduct thorough assessments of individuals' needs and develop tailored care plans.
  • Provide support and assistance with activities of daily living, including bathing, dressing, meal preparation, medication management, and mobility support.
  • Collaborate with healthcare professionals, social workers, and other team members to ensure a comprehensive and coordinated approach to care.
  • Monitor and evaluate individuals' progress, adjusting care plans as needed to promote optimal recovery and well-being.
  • Maintain accurate and thorough documentation of interactions, care plans, and progress.
Requirements
  • Previous experience in a similar care setting, preferably within a healthcare or social care environment.
  • Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals' lives.
  • Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
  • Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
  • Ability to work independently, demonstrating initiative and reliability.
  • Flexibility to work various shifts, including evenings, weekends, and bank holidays.
