Care Coordinator

We are seeking a highly skilled and experienced Care Coordinator to join our Early Intervention Team at Pennine Care NHS FT. As a key member of our team, you will play a vital role in supporting individuals who are experiencing a First Episode of Psychosis.
Key Responsibilities- Assess and manage complex caseloads, delivering comprehensive support for identified needs.
- Implement interventions in line with the psychosocial approach and recovery principles.
- Provide advice and supervision to other mental health professionals within the multi-disciplinary team.
- Contribute to the development of the service, promoting best practices and evidence-based interventions.
- Registered Mental Health Nurse (RMN), Occupational Therapist, or Social Work registration.
- Postgraduate qualification or equivalent experience relevant to the role.
- Experience of giving clinical supervision and working effectively in a multi-disciplinary team.
- Knowledge of early intervention for psychosis, psychosocial interventions, and NICE guidelines.
- Approved Mental Health Practitioner.
- Mentorship/Practice Leadership qualification.
- Experience in early intervention for psychosis, specialist placements, or research trials.
This is a full-time position, and the successful candidate will be required to use a car or have access to a means of mobility to travel across the Trust footprint.
Appointments to regulated and controlled activities require an enhanced DBS disclosure.
