About the Role

We are seeking a highly skilled and experienced Care Coordinator to join our team at Pennine Care NHS FT. As a Care Coordinator, you will play a vital role in providing high-quality care and support to individuals with complex mental health needs.

Key Responsibilities
  • Assess the needs of service users and develop personalized care plans
  • Coordinate and deliver care packages, including medication management and risk assessment
  • Work collaboratively with multidisciplinary teams to ensure seamless care delivery
  • Provide education and training to staff and service users
  • Contribute to the development and implementation of care policies and procedures
Requirements
  • Registered Mental Health Nurse, Occupational Therapist, or Social Worker
  • Current registration with the relevant regulatory body
  • Experience of working with individuals with complex mental health needs
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
