About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Lache Health Centre. As a key member of our multidisciplinary team, you will play a vital role in providing personalized care and support to our patients.

Key Responsibilities
  • Work closely with our clinical and administrative teams to ensure seamless care coordination.
  • Provide patient-centered support, including face-to-face and telephone interactions.
  • Administer tasks and duties with attention to detail and a focus on quality.
  • Coordinate patient recalls for health checks, QOF, and vaccinations.
  • Develop and maintain effective relationships with patients, families, and carers.
  • Collaborate with other stakeholders, including GPs, Practice Nurses, and healthcare professionals.
Person Specification
  • Experience working in healthcare or a related field, supporting vulnerable groups.
  • Ability to work in a multi-professional team environment.
  • Strong organizational skills, including planning, prioritizing, and time management.
  • Excellent communication and interpersonal skills.
  • Ability to actively listen, empathize, and provide personalized support.
  • Commitment to continuing personal and professional development.
