As a key member of our team, you will play a vital role in delivering high-quality patient-focused care to patients in their own homes.
About the RoleWe are seeking a highly motivated and forward-thinking registered nurse with substantial community experience to assist the District Nursing Team in the delivery of the Twilight element of the service as a triage nurse. The post holder will be required to work collaboratively with the wider integrated care teams, utilizing excellent communication and problem-solving skills to ensure seamless care delivery.
Key Responsibilities- Perform comprehensive assessments of patient nursing needs, planning, implementing, and evaluating care delivery according to changing health needs.
- Support the care management of complex/continuing care packages, ensuring practice is evidence-based through audit and critically appraising literature.
- Work collaboratively with other professionals within the integrated community team to ensure patient needs are met, involving patients/carers in the plan and delivery of care.
- Identify skills and knowledge to address individuals' training needs, undertaking personal development reviews and personal development plans.
- Participate in the health needs assessment of the practice population to identify areas of unmet need, utilizing risk assessment skills to ensure patient safety.
- Undertake chronic disease management reviews of housebound patients using the appropriate template, maintaining accurate records in accordance with the NMC standards.
- Registered General Nurse with a degree (or equivalent experience).
- ENB 998/City/Guilds 7307 or mentorship (or equivalent experience).
- Nurse Prescriber V100/V150.
- Extensive demonstrable post-registration experience, including care management and clinical leadership experience.
- Competent in a wide range of clinical nursing skills, with experience of successful multi-agency working and mentorship with a wide range of learners.
