About the Role

We are seeking a highly motivated and forward-thinking registered nurse with substantial community experience to join our dynamic district nursing team within the Ashton Leigh and Wigan District Nursing Service.

The successful candidate will be required to work collaboratively with the wider integrated care teams, providing high-quality patient-focused care to patients in their own homes.

Main Responsibilities

  • Perform comprehensive assessments of patient nursing needs, planning, assessing, implementing, and evaluating care delivery according to changing health needs.
  • Support the care management of complex/continuing care packages.
  • Ensure practice is evidence-based through undertaking audit and critically appraising literature to achieve a high standard of care.
  • Work collaboratively with other professionals within the integrated community team to ensure patient needs are met.
  • Involve patients/carers in the plan and delivery of care.
  • Identify skills and knowledge to address individuals' training needs by undertaking personal development reviews and personal development plans.
  • Participate in the health needs assessment of the practice population to identify areas of unmet need.
  • Ability to work without direct supervision and to solve problems.
  • Undertake chronic disease management reviews of the housebound patient using the appropriate template.

Requirements

  • Registered General Nurse qualification.
  • ENB 998/City/Guilds 7307 or mentorship (or equivalent experience).
  • Nurse Prescriber V100/V150.
  • Degree (or equivalent experience).
  • Evidence of CPD/Short courses Palliative care/diabetes/COPD/wound care.
