About the Role

We are seeking a dedicated and compassionate Enablement Support Worker to join our adult care department in Derbyshire County Council. As an Enablement Support Worker, you will play a vital role in supporting clients to regain full or partial independence in their own homes.

Key Responsibilities
  • Support clients in their own homes to achieve their goals and promote independence
  • Work collaboratively with a multi-professional team to provide wrap-around support
  • Develop and implement therapeutic goal-focused care plans
  • Provide regular updates and progress reports to clients and their families
  • Participate in ongoing training and professional development to enhance skills and knowledge
