Job Summary:

Derbyshire County Council is seeking a skilled Enablement Support Worker to join our adult care department. As an Enablement Support Worker, you will play a vital role in supporting clients to regain full or partial independence in their own homes.

Key Responsibilities:

  • Support clients in their own homes to regain independence in personal care, mobility, medication, and meal preparation.
  • Work collaboratively with a multi-professional team to provide wrap-around support and promote choice and control.
  • Develop and implement therapeutic goal-focused care plans to promote independence.
  • Provide regular updates and progress reports to clients and their families.
  • Participate in ongoing training and professional development to enhance skills and knowledge.

Requirements:

  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
  • Ability to maintain confidentiality and handle sensitive information.
  • Valid driving license and access to a vehicle.
